Welcome to the 2024 CIHA Resident Survey (Loussac Place)
Thank you for participating in this survey. We are committed to learning by listening. We hope to learn about your experience living in CIHA housing and we appreciate you taking the time to share your thoughts and feedback with us.
Instructions
- This survey takes about 15 minutes. Once you submit you cannot change your survey answers or retake the survey.
- Try to answer each question as completely as possible.
- Your responses are completely anonymous and confidential.
- Once you have completed the survey, click the “Submit” button.
- Once your survey is submitted you will receive a confirmation and an opportunity to enter to win a $50 VISA gift card in the CIHA Survey Raffle.*
1. Open to all CIHA residents ages 18 and older who submit a completed survey and then complete and submit a raffle entry.
- Residents who submit a completed Surveys by July 15th can enter their name 4 times into the raffle
- Residents who submit a completed Surveys by July 31st can enter their name 2 times into the raffle
- Residents who submit a completed Surveys by August 15th can enter their name 1 time into the raffle
2. A random computerized drawing will be held on September 6, 2024. Six winners will be selected from all eligible entries and will be awarded a $50 VISA gift card.
3. Winners will be contacted by email and phone on or about September 9, 2024 and prize delivery arrangements will be made at that time.
4. One prize per person.
