REQUEST FOR QUOTE – BAXTER FAMILY HOUSING PHASE II COUNTERTOP & WINDOWSILL MATERIALS

Cook Inlet Housing Authority (CIHA) is accepting sealed quotes hand-delivered, mailed, or
emailed from responsible and responsive vendors for the Baxter Family Housing Phase II –
Countertops & Windowsills Materials project, located at 4230 Baxter Road, Anchorage, Alaska.
SCOPE OF WORK
- The vendor shall furnish all solid surface countertop materials and related materials required for the Baxter Family Housing Phase II project in accordance with the quantities, colors, and product selections identified in the CIHA-provided quote submittal form and in compliance with applicable local, state, and federal codes and standards.
- Materials furnished under this Scope of Work include solid surface countertop sheet material, windowsill material, substrates, epoxies, silicone sealants, adhesives, and associated shop supplies as identified in the CIHA-provided quote submittal form.
- All solid surface materials shall be furnished exactly as identified in the CIHA-provided quote submittal form. No substitutions, alternates, or deviations are permitted without prior written approval from Cook Inlet Housing Authority.
- The vendor shall provide pricing for the materials identified in the CIHA-provided quote submittal form, including a cost breakdown by material category and a total material cost. Pricing shall be based solely on the quantities provided. Vendors are not required to perform a materials takeoff or verify field dimensions.
- Following award, Cook Inlet Housing Authority will issue a purchase order reflecting the approved materials and quantities. The vendor shall furnish and deliver materials strictly in accordance with that purchase order.
- The CIHA-provided quote submittal form is intended to establish a consistent basis for comparison between quotes. If discrepancies or omissions are identified during quote preparation, the vendor may submit written clarification with the quote proposal for CIHA’s consideration.
- The vendor shall deliver all materials to the Cook Inlet Housing Authority designated warehouse location, offload the materials, and place them in a protected manner as directed by CIHA. Materials shall be delivered in a condition that prevents damage from moisture or physical impact. Any shortages, damage, or discrepancies shall be reported at the time of delivery.
- The vendor shall provide manufacturer documentation for the furnished materials, including product data sheets, safety data sheets where applicable, and material warranties.
- This Scope of Work is limited to furnishing and delivering materials only. Templating, fabrication, cutting, installation, finishing, field modification, testing, and warranty labor are excluded unless expressly stated otherwise in CIHA-issued documents.
